Thanks everyone, especially @keithsw1111 and @Katekate. I feel stupid posting the PDF that showed the issue, we actually looked at it and didnt pick out the problem.... sometimes you cant see the forest for the trees!

something that is good to try to pick up any issues with your layout is to make a sequence that does horizontal and vertical bars over everything. any layout issues like this will be obvious.
